ABSTRACT

Abstract The evolution in imaging evaluation of musculoskeletal sarcomas contributed to a significant improvement in the prognosis and survival of patients with these neoplasms. The precise characterization of these lesions, using the most appropriate imaging modalities to each clinical condition presented, is of paramount importance in the design of the therapeutic approach to be instituted, with a direct impact on clinical outcomes. The present article seeks to update the reader regarding imaging methodologies in the context of local and systemic evaluation of bone sarcomas and soft tissues.


Resumo A evolução na avaliação por imagens dos sarcomas musculoesqueléticos contribuiu para melhora significativa no prognóstico e na sobrevida dos portadores destas neoplasias. A caracterização precisa destas lesões, mediante utilização das modalidades de imagem mais adequadas a cada condição clínica apresentada, é de suma importância no delineamento da abordagem terapêutica a ser instituída, com impacto direto sobre os desfechos clínicos. O presente artigo busca atualizar o leitor a propósito das metodologias de imagem no contexto da avaliação local e sistêmica dos sarcomas ósseos e das partes moles.

Introducción: Los tumores alrededor de la rodilla son frecuentes, a excepción de la rótula donde su localización es ocasional. En esta estructura anatómica existen características clínicas y epidemiológicas muy específicas según el tipo de tumor. Objetivo: Actualizar y brindar información sobre los tumores de la rótula. Métodos: La búsqueda y análisis de la información se realizó en un periodo de 62 días (primero de noviembre al 31 de diciembre de 2022) y se emplearon las siguientes palabras: patellar bone tumors, bone tumor AND patella, knee bone tumors a partir de la información obtenida se realizó una revisión bibliográfica de un total de 153 artículos publicados en las bases de datos: PubMed, Hinari, SciELO y Medline mediante el gestor de búsqueda y administrador de referencias EndNote, de ellos se utilizaron 30 citas seleccionadas para realizar la revisión, 28 de los últimos cinco años. Resultados: Se menciona la incidencia de los tumores benignos más frecuentes que afectan esta estructura anatómica, como el tumor de células gigantes. Se hace referencia a los tumores óseos malignos más reportados y su comportamiento clínico e imagenológico en especial el osteosarcoma. Se abordaron las metástasis de la rótula en particular las provenientes del pulmón. Conclusiones: Los tumores óseos de la rótula son lesiones infrecuentes, al comparar su incidencia con otros tumores de la rodilla. La presentación depende del tipo histológico y grado de afección ósea.


Introduction: Tumors around the knee are frequent, except for the patella, where its location is occasional. In this anatomical structure there are very specific clinical and epidemiological characteristics depending on the type of condition. Objective: To update and provide information on patella tumors. Methods: The search and analysis of the information was carried out in a period of 62 days (November 1st to December 31st, 2022) and the following words were used: patellar bone tumors, bone tumor AND patella, knee bone tumors. From the information obtained a bibliographic review was carried out, a total of 153 articles published in the PubMed, Hinari, SciELO and Medline databases through the EndNote search manager and reference manager were found, of which 30 selected citations were used to carry out the review, 28 of the last five years. Results: The incidence of the most frequent benign tumors that affect this anatomical structure, such as the Giant Cell Tumor, is mentioned. Reference is made to the most reported malignant bone tumors and their clinical and imaging behavior, especially Osteosarcoma. Metastases of the patella were addressed, particularly those of the lung. Conclusions: Bone tumors of the patella are infrequent lesions, when comparing their incidence with tumors from the rest of the knee joint. Presentation depends on the histological type and degree of bone involvement.

ABSTRACT Objective Present the preliminary results of a case series using the surgical ankle arthrodesis technique with an intramedullary retrograde nail for bone tumors. Methods We present the preliminary data of 4 patients, 3 males and 1 female, with a mean age of 46,2 (range 32 to 58) years, with histology proven Giant Cell Tumour of bone in 3 and osteosarcoma in 1. The mean resection length of distal tibia was 11,75 (range 9 to 16) cm, and all the patients underwent reconstruction with a tibiotalocalcaneal arthrodesis with an intercalary allograft fixed by a retrograde intramedullary nail. Results Oncological follow-up evolved without evidence of local recurrence or disease progression in all patients. After a mean time of 69.5 (range 32 to 98 months), patients had a mean MSTS12 functional score of 82.5% (range 75 to 90). All tibial arthrodesis and diaphyseal osteotomy sites were fused within 6 months with a return to activities without complications related to coverage skin or infection. Conclusion No complications were recorded; all arthrodesis and diaphysial tibial osteotomy sites fused by 6 months, and the mean follow-up of those patients was 69,5 (range 32 to 988) months, with a mean functional MSTS score of 82,5% (range 75-90). RESUMO Objetivo Apresentar os resultados preliminares de uma série de casos utilizando a técnica cirúrgica de artrodese do tornozelo com haste intramedular retrógada para tumores ósseos. Métodos Apresentamos os dados preliminares de quatro pacientes, três homens e uma mulher, com idade média de 46,2 (variação de 32 a 58) anos, com histologia comprovada de tumor de células gigantes em três e osteossarcoma em um. O comprimento médio de ressecção da tíbia distal foi de 11,75 (variação de 9 a 16) cm, e todos os pacientes foram submetidos à reconstrução com uma artrodese tibiotalocalcaneana com um aloenxerto intercalar fixado por uma haste intramedular retrógrada. Resultados O acompanhamento oncológico evoluiu sem evidências de recidiva local ou progressão da doença, em todos os pacientes. Após um tempo médio de 69,5 (variação de 32 a 98 meses), os pacientes tiveram uma pontuação média funcional MSTS12 de 82,5% (variação de 75 a 90). Todos os locais de artrodese e osteotomia diafisária tibiais foram fundidos em 6 meses com retorno às atividades de vida diária sem complicações relacionadas à cobertura ou infecção. Conclusão Não foram registradas complicações; todos os locais de artrodese e osteotomia diafisária da tíbia fundiram-se em 6 meses, e o acompanhamento médio desses pacientes foi de 69,5 (variação de 32 a 988) meses, com uma pontuação média funcional MSTS de 82,5% (variação de 75-90). ABSTRACT

ABSTRACT Psammomatoid juvenile ossifying fibroma (PJOF) is a benign fibro-osseous lesion that mainly affects the paranasal sinuses and periorbital bones. It may cause significant esthetic and functional impairment. Herein, we describe the diagnosis and surgical approach of an extensive PJOF arising in the frontal sinus of a young male. After complete lesion removal and histopathological confirmation, the bone defect was repaired with a customized polymethylmethacrylate implant. PJOF may present aggressive clinical behavior. The excision of extensive PJOF in the orbitofrontal area can result in significant esthetic defects. Polymethacrylate implants restore functionally and esthetically the involved area.

ABSTRACT

Abstract Bone and soft tissue sarcomas consist of a heterogeneous group of malignant tumors of mesenchymal origin that can affect patients from any age. The precise staging of these lesions determines the best therapeutic strategies and prognosis estimates. Two staging systems are the most frequently used: the system proposed by the University of Florida group, led by Dr. William F. Enneking (1980) and adopted by the Musculoskeletal Tumor Society (MSTS), and the system developed by the American Joint Committee on Cancer (AJCC) (1977), currently in its 8th edition (2017). This paper updates the reader on the staging of bone and soft tissue sarcomas affecting the musculoskeletal system.


Resumo Os sarcomas ósseos e das partes moles consistem em grupo heterogêneo de neoplasias malignas de origem mesenquimal que podem ocorrer em qualquer faixa etária. O estadiamento preciso destas lesões determina as melhores estratégias terapêuticas e estimativas de prognóstico. Dois sistemas de estadiamento são os mais frequentemente empregados no manejo destas neoplasias: o sistema proposto pelo grupo da Universidade da Flórida, liderado pelo Dr. William F. Enneking (1980), adotado pela Musculoskeletal Tumor Society (MSTS) e o sistema desenvolvido pela American Joint Committee on Cancer (AJCC) (1977) que se encontra em sua 8a edição (2017). O presente artigo busca atualizar o leitor a respeito do estadiamento dos sarcomas ósseos e das partes moles que acometem o sistema musculoesquelético.

SUMMARY OBJECTIVE In this study, we aimed to investigate the role of COL6A3 on cell motility and the PI3K/AKT signaling pathway in osteosarcoma. METHODS The relative expression of COL6A3 was achieved from a GEO dataset in osteosarcoma tissue. siRNA technology was applied to decrease the COL6A3 expression in cells, and cell counting kit-8 (CCK-8) assay and colony formation analysis were used to examine the cell proliferation potential. Knockdown COL6A3 made the proliferation and colony formation abilities worse than the COL6A3 without interference. Likewise, in contrast to the si-con group, cell invasion and migration were inhibited in the si-COL6A3 group. Moreover, the western blot results suggested that the PI3K/AKT signaling pathway was manipulated by measuring the protein expression of the PI3K/AKT pathway-related markers, due to the COL6A3 inhibition. CONCLUSION COL6A3 plays a crucial role in modulating various aspects of the progression of osteosarcoma, which would provide a potentially effective treatment for osteosarcoma.


RESUMO OBJETIVO Neste estudo, investigamos a função do COL6A3 na mobilidade celular e na via PI3K/AKT em osteossarcomas. METODOLOGIA A expressão relativa do COL6A3 foi obtida a partir de dados GEO em tecidos de osteossarcoma. O RNA de interferência (siRNA) foi utilizado para reduzir a expressão do COL6A3 nas células, e o teste de contagem de células kit-8 (CCK-8) e a análise de formação de colônias foram realizados para examinar o potencial de proliferação celular. Além disso, o Transwell comprovou os efeitos do si-COL6A3 na invasão celular e migração em células de osteossarcoma. Para medir os níveis de expressão das proteínas e mRNAs, utilizamos transcriptase reversa quantitativa (qRT-PCR) e western blot. RESULTADOS O COL6A3 foi regulado nos tecidos e células do osteossarcoma quando comparado com o controle normal. A redução de COL6A3 reduziu a proliferação e a capacidades de formação de colônias em relação ao COL6A3 sem interferência. Do Mesmo modo, ao contrário do observado no grupo si-con, a invasão e migração celular foram inibidas no grupo si-COL6A3. Além disso, o resultado do western blot sugere que a via PI3K/AKT foi manipulada, medindo a expressão proteica dos marcadores relacionados à PI3K/AKT, devido à inibição do COL6A3. CONCLUSÃO O COL6A3 desempenha um papel crucial na modulação de vários aspectos da progressão do osteossarcoma, o que pode representar um possível tratamento eficaz para a doença.

RESUMEN Fundamento: el cáncer constituye una causa frecuente de morbi-mortalidad a nivel internacional y nacional. Dentro de las formas de presentación se encuentran las metástasis óseas. Objetivo: profundizar los conocimientos de la enfermedad ósea metastásica enfocada en el manejo desde el punto de vista de la especialidad de Ortopedia y Traumatología. Métodos: la búsqueda de la información se realizó en un periodo de tres meses (primero de enero de 2018 al 31 de marzo de 2018) y se emplearon las siguientes palabras: bone metastasis, pathologic fracture, y metastasic bone disease, a partir de la información obtenida se realizó una revisión bibliográfica de un total de 259 artículos publicados en las bases de datos PubMed, Hinari, SciELO y Medline mediante el gestor de búsqueda y administrador de referencias EndNote, de ellos se utilizaron 50 citas seleccionadas para realizar la revisión, de ellas 41 de los últimos cinco años, se incluyeron diez libros. Desarrollo: se describen las metástasis óseas de acuerdo a su aspecto radiográfico en blásticas, líticas y mixtas; así como los principales sitios de origen. Se hace referencia a los principales estadios, con especial énfasis en el riesgo de fractura patológica, para lo que se plasman criterios, que permiten definir la conducta a seguir. Se mencionan las indicaciones y variantes del tratamiento quirúrgico, así como el pronóstico de estos enfermos. Conclusiones: la enfermedad ósea metastásica es un reto para el cirujano ortopédico, tiene indicaciones quirúrgicas específicas y uno de los elementos de más importancia es definir el riesgo de fractura patológica.


ABSTRACT Background: cancer is a common cause of morbidity-mortality affecting many people internationally and nationally. One way of presentation is bone metastases. Objective: to deepen the knowledge about the management of patients suffering from bone metastasis from a point of view of the Orthopedics and Traumatology specialties. Methods: a search of information was carried out in a period of three months (January 1st, 2018 to March 31st, 2018) in the databases PubMed, Hinari, SciELO and Medline through the information locator EndNote by using the words bone metastasis, pathologic fracture, and metastatic bone disease, resulting in a total of 259 articles which 50 of them were selected for the review, 41 of them of the last five years, including ten books. Development: bone metastasis types were described according to radiographic aspects as: litic, blastic and mix, so like the most common origin sites. The main stages were shown, specially the one related to the risk of pathologic fracture, so that criteria were stated to define treatment. Indication and treatment modalities were described, as well as prognostic factors. Conclusions: metastatic bone disease is a great challenge for orthopedic surgeon, there are specific indications for surgery and one of the most important aspects to take in mind is the risk of pathologic fracture.

Abstract Objective: The purpose of this study was to determine whether chemical-shift magnetic resonance imaging (MRI) could be useful in the diagnosis of osteoid osteoma when clinical and radiological tumor features are inconclusive. Materials and Methods: This retrospective study included 17 patients who underwent chemical-shift MRI for the evaluation of osteoid osteoma. For all patients, two musculoskeletal radiologists independently recorded signal intensities on in-phase and out-of-phase images in the nidus of the tumor, in abnormal-intensity bone marrow surrounding the lesion, and in normal-appearing bone marrow. For each region, relative signal intensity ratios were calculated by dividing out-of-phase by in-phase values. Relative ratios > 1 were considered indicative of neoplastic lesions. Statistical analysis was carried out to analyze the sample. Inter-observer and intra-observer agreement for each imaging method were assessed using intraclass correlation coefficients according to the Fleiss method and a value > 0.65 was considered to indicate substantial agreement. Results: The mean relative signal intensity ratios were 1.2 (range, 0.9-1.4) for the nidus and 0.35 (range, 0.11-0.66) for the surrounding tissue; these values differed significantly from the relative signal-intensity ratios for normal-appearing bone marrow (p < 0.05). Conclusion: Chemical-shift MRI is useful for the diagnosis and evaluation of osteoid osteoma.


Resumo Objetivo: O objetivo deste estudo foi determinar se a sequência T1 em fase e fora de fase da ressonância magnética (RM) poderia ser útil no diagnóstico de osteoma osteoide em situações nas quais características clínicas e radiológicas são indefinidas. Materiais e Métodos: Este estudo retrospectivo incluiu 17 pacientes submetidos a RM para avaliação do osteoma osteoide. Em todos os pacientes, dois radiologistas musculoesqueléticos registraram, independentemente, as intensidades de sinal em imagens em fase e fora de fase no nidus do tumor, na medula óssea de intensidade anormal ao redor da lesão e na medula óssea de aspecto normal. Para cada região, as relações de intensidade de sinal relativas foram calculadas dividindo os valores na sequência fora de fase pelos valores em fase. Razões relativas > 1 foram consideradas indicativas de lesões neoplásicas. A análise estatística foi realizada para analisar a amostra. A concordância interobservador e intraobservador para cada método de imagem foi avaliada por meio dos coeficientes de correlação intraclasse, segundo o método de Fleiss, e considerou-se um valor > 0,65 para indicar concordância substancial. Resultados: As razões de intensidade de sinal relativa média foram 1,2 (variação: 0,9-1,4) para o nidus e 0,35 (variação: 0,11-0,66) para o tecido circundante. Estes valores diferiram significativamente das relações de sinal-intensidade relativa da medula óssea com aspecto normal (p < 0,05). Conclusão: A sequência em fase e fora de fase da RM mostrou-se útil para o diagnóstico e avaliação do osteoma osteoide.

ABSTRACT

El sarcoma de Ewing es una neoplasia maligna que afecta fundamentalmente a niños y adolescentes. Cerca del 95 % de estos sarcomas ocurre entre los 5-25 años. Es más frecuente en el sexo masculino y muy raro en la raza negra. Tiene predilección por las diáfisis de los huesos largos, las costillas y los huesos planos, como la escápula y la pelvis. Se presenta a una paciente de 37 años de edad con lesión tumoral en la mano la izquierda. Como tratamiento de primera línea se le realizó amputación del primer y segundo dedos de la mano. La biopsia informó sarcoma de Ewing. Posteriormente la paciente fue sometida a esquema de quimioterapia. Se considera de interés la presentación de este caso por la localización poco usual de la neoplasia.


Ewing’s sarcoma is a malignant tumor that primarily affects children and adolescents. Nearly 95% of these sarcomas occur between the ages of 5 to 25 years. It is more common in males and extremely rare among blacks. It presents most often in the diaphyses of long bones, the ribs and flat bones, such as the scapula and pelvis. We present the case of a 37 year-old woman with a tumor in the left hand. The amputation of the first and second fingers was performed as first-line treatment. The biopsy revealed a Ewing’s sarcoma. Subsequently, the patient underwent chemotherapy regimen. This case is of interest given the unusual location of the tumor.

Recent studies show that the abnormalities of dickkopf-1 (DKK-1)are closely related to the bone metastasis and therapeutic effect of breast cancer,prostatic carcinoma,multiple myeloma,neuroblastoma and non-small cell lung cancer.Clinical researches of large sample on the relationships between serum DKK-1 concentration and malignant tumors bone metastases have important clinical significance for evaluating the value of DKK-1 as a clinical biological biomarker of bone metastases and understanding the tumor mechanisms of bone metastases.

Fueron evaluados 25 pacientes que acudieron a la Unidad de Tumores Óseos y Partes Blandas (UTO-PB) del Hospital "Manuel Núñez Tovar" entre enero de 2002 a enero de 2008, con fracturas de la región proximal del fémur, provocadas por lesiones metastásicas de carcinomas, predomino en el estudio el sexo masculino, las edades de los pacientes estuvieron por encima de los 40 años, con mayor numero en la década de los 50, el cáncer mas importante en el hombre fue el de próstata y en la mujer el de cuello uterino. En todos los procedimientos se seleccionó pacientes con expectativa de vida superior a 4 semanas y se persiguió como objetivo fundamental mejorar calidad de vida, aliviar dolor y reincorporar al paciente a su estilo de vida previo a sufrir la fractura. Se realizó artroplastia para lesiones cervicales y osteosíntesis con clavos intramedulares y sistemas de placas con tornillos deslizantes para la región subtrocantérica e intertrocantérica, obteniéndose mejores resultados funcionales con las artroplastias y menor índice de complicaciones con respecto al uso de clavos intramedulares y a los DCS, DHS.


We evaluated 25 patients who came to the Unit of Bone and soft tissue tumors (UTOPB) Hospital "Manuel Núñez Tovar" between january 2002 and january 2008, with fractures of the proximal femur, lessions caused by metastic carcinoma, predominance in the study males, ages of the patients were above age 40, with the highest number in the 50s, the most important cancer in men was prostate cancer and women in the cervical. All procedures were selected patients with life expectancy greater than 4 weeks and was pursued as a key objective to improve quality of life, alleviate pain and return patients to their lifestyle prior to suffering the fracture. Arthroplasty was performed for cervical lesions and osteosynthesis with intramedullary nails and screws systems with sliding plates for the region subtrocanterica and intertrocanterica, obtained better functional results with arthroplasties and lower complication rate with respect to the use of intramedullary nails and DCS, DHS.

0bjective To evaluate the application of whole body MR diffusion weighted imaging (DWI)in the detection of bone metastasis using skeletal scintigraphy as the referenee.Methods Fonv.two healthy volunteers and 38 patients with malignant tumors were enrolled in our studv.A11 the patients received MR examination and skeletal scintigraphy within one week.MR examination was performed on GE signa 3.0T MR scanner using a build.in body coil.The skeletal system Was divided into eight regons and the images of the whole body MR DWI and skeletal seintigraphy were reviewed to compare the two modalities patient by patient and region by region.The images were reviewed separately by two radiologists and two nuclear medicine physicians,who were blinded to the results of another imaging modality.Results A total of 169 metastatic lesions in 69 regions of 30 patients were detected by whole body MR DWI while 156 lesions in 68 regions of 29 patients were identified by skeletal seintigraphy.There were two cases negative in scintigraphy but positive in whole body MR DWI and one case positive in scintigraphy only.There were eight lesions negative in scintigraphy but positive in whole body MR DWI,mainly located in the spine.pelvis and femur.Seven 1esions were only detected by scintigraphy,mainly located in the skull.sternum.clavicle and scapula.Conclusion The whole body MR DWI reveals excellent consistency with skeletal scintigraphy regarding bone metastasis.and the two modalities are complementary for each Other.

ABSTRACT

Objective To evaluate the clinical application of contrast-enhanced digital subtraction MRI in musculoskeletal malignant tumors.Methods Fifty patients with musculoskeletal malignant tumors were scanned by contrast enhanced T 1WI with an injection of Gd-DTPA (0.1 mmol/kg), and digital subtraction was performed between post-contrast-enhanced images and pre-contrast-enhanced images. Musculoskeletal malignant tumors were evaluated by means of a contrast-to-noise ratio (C/Ns). Imaging sign of the musculoskeletal malignant tumors were compared between subtraction MR images and conventional contrast enhanced MR images.Results Contrast-enhanced digital subtraction MR images were better than conventional contrast enhanced MR images. All cases were confirmed by operation or pathology. All the lesions had higher C/Ns on subtraction MR imaging than on conventional contrast enhanced MR images.
